SCHEDULE: 40 hours, Monday - Friday, 6:45 p.m.- 3:15 p.m.

Pays $19.00 per hour

JOB DESCRIPTION:

The Case Manager will be responsible for the overall case management of the participants in the program at 8 Church Street.  This program will follow the models of Housing First/Safe Haven.  The Case Manager will use harm-reduction, trauma-informed approach and Motivational Interviewing techniques to create and maintain a respectful atmosphere that empowers very chronic, mentally ill participants to reach their highest level of functioning.  Participants will be assessed for their actual level of functioning in various spheres upon entering the program, and assisted in individual plans to improve and enhance this level.  The Case Manager is required to provide services to these individuals with the goals of housing stability and psychosocial rehabilitation; advocating for appropriate mental health services.  Due to emergency or unforeseen program needs, staff may be temporarily or permanently reassigned to another Safe Haven program at any given time



Requirements

REQUIREMENTS:

Education/Training:

  • Associate’s Degree
  • A valid Massachusetts Driver’s License in good standing

Knowledge/Experience:

  • Minimum of three (3) years prior work experience in a therapeutic setting, community residence or in an inpatient setting 
  • Experience as a mental health team member
  • Familiarity with the concepts of psychosocial rehabilitation and consumer empowerment
  • Experience conducting psychosocial assessments
  • Experience with case management and implementing treatment plans
  • Knowledge of treatments for chronic mental illness, including medications
  • Previous experience working with dually diagnosed individuals
  • Experience assisting people with ADL skills
  • Experience working with other vendors and services within the public mental health system
  • Previous experience moving homeless mentally participants into housing

Physical Abilities:

  • Ability to attend outside meetings
  • Ability to respond to individuals in crisis
  • Ability to climb several flights of stairs quickly and frequently
  • Ability to accompany tenants to appointments
  • Ability to assist certain individuals with ADL skills

Mental Abilities

  • Ability to multi-task and set priorities

  • Ability to resolve conflicts diplomatically
  • Demonstrate supportiveness and nurturance
  • Ability to set limits when needed, and observance of appropriate boundaries

  • Willingness to use continuing education as a tool to gain ideas for improving client outcomes

Preferred:

  • Bachelor’s Degree in nursing or other related field such as counseling, psychosocial rehabilitation, or psychology
  • Advanced training in psychosocial rehabilitation and substance abuse
  • Bi-lingual – Spanish/English
  • Previous experience working with a Housing First/Safe Haven model
  • Familiarity with the Metropolitan Boston public and private mental health system
  • Long term treatment experience with chronically mentally ill people

Essential Functions Include But Are Not Limited To:

  • Carry a case load and meet regularly with each individual client in caseload for the purpose of addressing barriers to housing retention.
  • Work with participants to create client–centered, strengths- based, individual treatment plans to be periodically evaluated and edited as necessary/appropriate and/or according to program standards. Perform ongoing clinical assessments for participants in caseload 
  • Write monthly DMH reports based on treatment goals of participants, providing a clear clinical picture
  • Confer daily with other program staff to promote program integrity and to proactively identify any group issues that need addressing
  • Educate, coach, and reinforce activities of daily living (ADL) skills, including, but not limited to personal hygiene, budgeting, cooking and leisure time activities.  Assist participants in shopping, laundry, and other ADL as needed or requested
  • Meet regularly with each individual client to seek opportunities for ongoing psychosocial rehabilitation
  • Work with the Case Manager Supervisor to evaluate client progress
  • Negotiate with other departments around the Inn to facilitate successful case management outcomes
  • Work cooperatively with other agencies to secure needed levels of support for individual participants
  • Set a tone of respect and support for all participants of the program
  • Work with participants in caseload to teach and reinforce good ADL skills, including but not limited to personal hygiene, budgeting, cooking, and leisure time activities
  • Assist participants in shopping, laundry, and other activities of daily living as needed or requested
  • Guide the chronically mentally ill participants in an intentional progression towards increased autonomy, improved life skills, and enhanced quality of life
  • Involve participants in milieu to enhance connectedness with life satisfactions and responsibilities.
